JATC Claim Instructions and Forms
If you have an accident that results in medical expenses, contact your Local’s administrative office. You will have to complete a claim form and submit with itemized medical bills to:

Claim payments come directly from CHARTIS (the insurance company) and generally take about 4 weeks from the time the completed claim is received.
