Accident Medical Program

This program is designed for day care centers, nursery schools, preschools, play schools,
Montessori schools, kindergartens, before/after school care and similar institutions.

Features:

    Mandatory coverage for all children - No rosters required.

    Coverage is provided while traveling directly to and from, and while participating in
    any supervised and sponsored activity.

    No restrictions as to the number of insureds - $200 minimum premium.

    Various medical maximums, deductibles, and benefits available.

    Coverage also available for teachers, aides and administrative personnel.

Benefits:

    Up to $25,000 accident medical expense maximum

    No deductible

    Reasonable coverage expenses are payable in full subject to the following limitation

        a.  Hospital Room & Board - Semi-private room rate.

        b. Dental - $100 per sound and natural tooth.  $500 maximum per injury.

        c. Outpatient Physical Therapy - $100 Maximum.

        d. Outpatient Orthopedic Appliances and Braces - $100 maximum.

 

Treatment must commence within 30 days from the date of injury.

Coverage up to one year from the date of injury.

Coverage is excess to other insurance.
