Insurance Plan - Benefit Summary

The Accident Insurance Plan provides insurance for Apprentices and Trainees while participating in, including direct travel to and from, apprenticeship and training classes and meetings sponsored by the participating Local.

Option I:

$25,000	Accident Medical Expense Benefit (after a deductible of $25 per accident)
$25,000	Accidental Death & Dismemberment Death Benefit
$25,000	for the loss of either two hands, two feet, two eyes, or any combination of any two of the above.
$12,500	for the loss of either one hand, one foot, or one eye

Option IB:

$25,000	Accident Medical Expense Benefit (after a deductible of $25 per accident)
$25,000	Accidental Death & Dismemberment Death Benefit
$25,000	for the loss of either two hands, two feet, two eyes, or any combination of any two of the above.
$12,500	for the loss of either one hand, one foot, or one eye
$200	Weekly Accident Indemnity Benefit (maximum 52 weeks)

Option IC:

$50,000	Accident Medical Expense Benefit (after a deductible of $25 per accident)
$50,000	Accidental Death & Dismemberment Death Benefit
$50,000	for the loss of either two hands, two feet, two eyes, or any combination of any two of the above.
$25,000	for the loss of either one hand, one foot, or one eye
$200	Weekly Accident Indemnity Benefit (maximum 52 weeks)

If you have an accident that results in medical expenses, contact your Local's administrative office. You will have to complete a claim form and submit with itemized medical bills to:

Claim payments come directly from AIU (the insurance company) and generally take about 4 weeks from the time the completed claim is received.
