HOW UNITED CONCORDIA WORKS FOR YOU
The delivery of dental services hasn't changed much over the years. Until United Concordia. United Concordia was created by a team of professionals who saw the growing need for quality, convenient and affordable dental care. Our solution? An HMO-style dental benefit program designed to meet the need for practical, cost-effective coverage. In addition to cost savings, we maintain a vigorous program of quality assurance in an effort to enhance the value of services you receive. Under this program, we carefully select the doctors who become part of our network. We periodically inspect offices and facilities. Our Dental director and staff of associates review charts on a random basis to gauge the general quality of care being delivered by our independently contracted providers.	We audit office policies to determine that patients are being given appointments on a timely basis and that emergency services are in place, such as for the treatment of a weekend or holiday toothache. We have a program of pre-authorization to double check that any specialized care being recommended for you is appropriate. In large part, we exist to eliminate over-treatment or unnecessary treatment. Our ultimate goal is preventative care, which can be especially effective in dentistry where many dental problems can be avoided or solved with proper maintenance. Using United Concordia is easy. Simply present your United Concordia ID card at the time of service. There are no claim forms to worry about. Pay the related co- payment for the services you received. That's it, you're finished. Enrolling in United Concordia is as easy as 1, 2, 3.

1. Review the following Schedule of Benefits (ADA Code # with Description of Services and the Participant Copay).
   
2. Select a participating general dentist from the Provider Directory - remember to select a site for each member of the family.
   
3. Choose payment method and print appropriate forms:


Payroll Deduction - choose:Direct Deposit Form or  Employee Express/HR Links Format.

Direct Deposit Form is mailed to your payroll office.

Employee Express/HR Links Format provides information for your online connection to your payroll office.

Monthly Check Service - include the Monthly Check Service Form

Quarterly Direct Bill - upon receipt of your Enrollment Form, we will mail you a "Notice of Payment Due@ invoice.
